二、不同波长LED光的功效差异

2.2 蓝光(400-495nm):抗菌与控油

- 杀灭痤疮丙酸杆菌(Cutibacterium acnes)

作用机制:
痤疮丙酸杆菌产生的内源性卟啉(主要是粪卟啉III)在蓝光照射下被激活,产生单线态氧,从而杀灭细菌。
临床证据:
一项系统评价和荟萃分析纳入了多项随机对照试验,结果显示蓝光治疗可显著减少炎症性痤疮皮损数量,疗效与口服抗生素相当,且无明显副作用。2023年的一项动物实验确定了蓝光治疗的最佳参数为100 mW/cm²的功率密度。
但需注意: 蓝光并非对所有人都适用。有研究指出,蓝光可能产生氧化应激,对皮肤造成潜在损伤,尤其是在高剂量暴露时。此外,蓝光对深层痤疮的效果有限,主要作用于皮肤表面。
2.3 黄光(570-590nm)与绿光(520-560nm)

三、LED光疗的安全性与局限性

与紫外线不同,LED光疗不引起DNA损伤,无光致癌风险。

- 需要多次治疗:通常需要数周至数月的持续治疗才能看到明显效果
- 家用设备功率限制:家用LED设备功率远低于医用设备,疗效可能有限
